Rates of Service and Payment

Specialized Services
Request for documentation including but not limited to letters, summary of services, medical leave, court documentation, FMLA or disability forms are charged at $80 per hour.

Sliding Scale Fees
Sliding Scale Fees: Available based on income and need (limited availability)

Additional Fees
• Late Cancellation (less than 24 hours’ notice): $50
• No-Show Fee: Full session cost

Rates Subject to Change
Session rates are subject to review and potential adjustment annually, and they may also vary based on changes in the client’s income level. Clients will be notified of any rate changes at least 30 days in advance to ensure transparency and allow for planning.

Payment Policy:

CCM accepts cash, checks, and credit card.

Financial Assistance:

1. Attendees of our church partners may receive financial assistance through church benevolence, please speak with your pastor about your need.
2. A client who requires financial assistance may be eligible for our needs-based assistance program. For those who qualify, an adjusted rate and number of sessions may be provided for counseling services. To determine eligibility, please download our Financial Assistance Form. 

Insurance:

Some insurance companies will provide out-of-network reimbursement for counseling fees. Filing out-of-network claims is your responsibility; please contact your insurance company directly to determine documentation needed to file such a claim.
CCM does not accept any insurance plans.
